Como usar este questionario

Use este questionario para anotar la frequencia que Ud. Ve al nino comportarse en las maneros que se describen en la pagina seguiente. Antes de empezar, lea la pagina entera que da la lista de comportamientos asi se asegura que es lo que Ud. debe observar en el nino.
Observe al nino por un periodo de seis horas en su horario normal de todos los dias, por ejemplo en su hogar o en el lugar donde lo cuidan diariamente. No se apresure,
Use el rating scale en la pagina siguiente circule el numero que indica si su nino se comparta (puedo que no se comporte) de las que se describen.

Sume todos los numeros que ud. Marco en circulo y ponga el total en el cuadrado.
Un total entre 0 – 10 indica que No hay nececidad de referir al nino a mas tests.
Un total entre 11 – 25 indica que es existe una situacion leve y se debe conversar con el medico y posiblemente mas tests.
Un total de mas de 25 indica una necesidad inmediata de referencia Medica y posible intervencion.
